About UVM: Porter Medical Center VWWP is a volunteer-run initiative which connects newcomers to Vermont, with existing residents, to help you get acclimated and access to resources. The concept is simple – each “Participant” is paired with two to three “Hosts” for one-on-one meetings. Over a coffee, beer, or a little Vermont adventure, participants are welcomed and learn ideas about making the most out of life here. There is so much going on across our great state and we want to help people get plugged in to it all. Hosts and Participants sign up to join the program in their local area via a short online form. Hosts and Participants are then paired up based on location, interest, and availability. Additional Assignment Details: Facility Name: UVMHN Porter Medical Center Total # of Facility Beds: 25 Unit Name and Cost Center: UVMHN Porter Medical Surgical Telemetry Unit Leader/Point of Contact: Erica Bedell and Elizabeth Wojciechowski Unit Specialty: Medical Surgical Telemetry and some Progressive Care Trauma Level: Critical Access Hospital Minimum # Years Clinical Experience Preferred: 2 years Minimum # Years Clinical Experience in Specialty Required: 2 years Medical Surgical Tele Certifications Preferred: ACLS, BLS Required Nurse Patient Ratio by shift: by acuity but gen 1- 4 or 5 Circulate/Scrub as applicable: N/A Ancillary Assistance: staff per shift: 2-3 or more at times as nee #staff per shift: varies day to day Other: Hospital Volunteers Specialty Testing for Unit/Facility: U F #staff per shift: varies by availability Patient Care Modality: Patient Centered Documentation System: EPIC Medication System: PYXIS To what units/facilities: Totally voluntary w/in scope Describe the rotation: only voluntary Scheduling: 12H 645-715 am/pm Holiday Requirements: As needed to staff unit Patient Population (Diagnoses): Med Surg Tele pts,orthos,urology,ENT,GYN, Gen Surg,PC Patient Age Range: Young adult to end of life Special Equipment: (For example: IV Pumps: Type; Invasive Monitoring; Monitors: Type; ECMO; Ventilators: Type; ABG: Type) Alaris, Phillips Cardiac monitoring Who manages ventilators: Respiratory-they also manage all nebs and inhalers. Unit Specific Procedures and Protocols: (For example: Drips; Infusions; Phlebotomy; Starting IVs; EKGs; Transports; Weaning; Nebulizer to Inhaler; Starting IVs, performing venipuncture, Performing EKGs; Discontinuing sheaths) We are small but mighty. As such we are the Code 99,Rapid Response, Fall and IV team. We care for the usual mixture of medical patients, telemetry patients and our surgical patients include orthopedics, urology, some gyn and general surgery. We also care for some progressive care level patients. These patients would be those who are thought to need a progressive level of care for just a short time but not felt to be unstable or imminently needing ICU level of care. Those more intense patients would be transferred to an ICU/ER within network. We do have written protocols in place to provide supportive care while transfer arrangements are being made. We would never ask a staff member to work outside of their skill set but would provide support and an opportunity for learning. Of note we have 24/7 hospitalist coverage dedicate solely to our unit. This group is very approachable and supports a culture of teamwork and professional growth. They value our nursing team and our contributions to excellent patient care. Additional Unit Specific Information: As a Critical Access Hospital our current max unit census is 25 within17 rooms. There are 4 private rooms (1 End of Life room and 3 isolation rooms). Other semi-private rooms are set up to support negative pressure as well. When we as managers are not present there are house supervisors who watch over all of the units. One of us is always on call and will come in to provide support when needed. We have 24/7 lab and xray support as well. We are looking for experienced critically thinking staff members who are able to integrate quickly into our team to support our mission, vision and values.
