Shift Breakdown: 5x8 hrs
Shift: 8 hours, days

8 am-5 pm, Monday-Friday. Rehabilitation skills both individually and in group settings on-site and at local housing developments. Teaching skills, psychoeducation, symptom management, medication management, independent skills. Experience working with unhoused and at-risk persons served. Case types: Complete CalAIM assessments Diagnosis and documentation Development of treatment plans Maintenance of progress notes inclusive of homelessness, antecedents prior to homelessness, and other barriers Field-based work in-person with tenants Engagement with unhoused individuals not yet linked to services Completion of WIC 5150 applications for psychiatric hold as appropriate Therapeutic case management Linkage to resources and support in completing documents necessary to obtain services and benefits Psychotherapy both in-person and via telehealth as appropriate Transportation supporting case management Attend meetings for updates on housing-related topics Understand legislation related to housing Develop and maintain relationships with property managers, understanding the roles and responsibilities Being present in the community living space and engaging persons served/tenants with or without requests from the person served
