Day Shift 7 am- 3pm Evening Shift 3pm- 11pm Night Shift 11pm- 7am Summary/Objective: In keeping with our organization's goal of improving the lives of the Resident's we serve, the LPN provides supervision of staff and safeguards the health, safety, and welfare of all Residents under their care by following applicable laws, regulations, and established nursing policies and procedures. Qualifications: High school diploma or equivalent. Current State License as a Licensed Practical Nurse. LTC/SNF Experience Preferred Unit Profile/Facility Name: Wilson Rehabilitation and Nursing CenterFloor Specialty: Long Term and Short Term Care Number of Openings: Shift: Day 7-3, Evenings 3-11, Night 11-7 Weekly Guaranteed Hours: 40 Weekend Hours: Every other weekend Required Certifications: AHA BLS Years of Experience: 1+ Float Requirements: n/a Additional Notes: Unit Information: Number of Beds: 90 Patient Ratios: Days 1:15-23, Evenings 1:15- 23, Nights 1:24-30 Equipment: Hoyer Lifts, Sit-to-Stands, kangaroo pumps, IV pumps, Oxygen Tanks and Concentrators, Glucometers EHR: Point Click Care Supervisor: Onsite Mon-Fri 24 hours (except on Holidays and RTO), Charge Nurse Onsite Sat/Sun 7am 11pm, On Call Administrative RN Every Weekend Patient Population: Geriatric and adult Hiring Profile: Must Have Skills/Experience: Medication administration, Vital sign interpretation Preferred Skillset/Experience: PCC, Tube Feed Administration, IV Medication Administration, Nebulizer Administration Common Diagnosis/Treatment: CHF, Renal Disease, Dialysis, Diabetes, Post-surgical (femur fractures, hip fractures, abdominal surgeries, amputations), Dementia, Chronic Wounds, Stroke, UTI Personality Fit: Friendly, able to communicate with coworkers and residents, dependable, accountability
