recent 3 years + LPN SNF experience required Department: LTC | LPN - Type of Facility: SNF 60/LTC 35-50. - # of Beds/Residents: Census is typically 80-95. 20 Rooms are not private. - EMR: PCC - Ratios: RN's - LTC 1:25. SNF 1:13. LPN's - LTC 1:25. SNF 1:13. CNA's - 1:10-15 ( try to keep around 10). - Required Certifications: BLS - Common diagnosis: Feeding tubes, IV ABX, post surgicals, rehab, LTC. - Job Duties: IV, assessments, medications, dressing changes, wound vacs. RT in house during the day M-F- otherwise nurses do treatments. Admission/discharges completed by the RN. CNAs do vitals/ADLs, no blood sugars. - Experience: One year of experience. Would like experience with geriatrics. First time traveler okay. - Are there IV's or titratable drips?: IV abx/fluids. No drips. - Are RN's required to attend team conferences for acute inpatient rehab: Encouraged to do grand rounds/ meetings. - DON/Charge Nurse: 2 - ADONS. Charge nurse- RN. 2 dedicated charge nurses on nights. Take a full patient load. - Physician availability after hours: Hospitalist from the hospital covers. NP during the day M-F. Some private MDs follow their patients. They have an on call provider 24/7. - Interdisciplinary Team: PT/OT/SLP, Pulmonary or Cardiac Rehab, Case Managers, Social Workers, Dieticians? PT/OT/ SLP, RT, social worker, MDS nurses, wound care nurse, dietician in house. - Is RT on-site 24/7: AVAP/CPAP/ BIPAP- mostly if they are on at night. They do have times where the patient can be on continuous. - Pharmacy on-site: Dedicated pharmacy- available 24/7. Omnicell and med carts- for everyday medications. Med cart for day to day use. Omnicell used for new patients or new orders if they aren't loaded in the cart. - Shifts & Scheduling: 5-5 For RN/LPN- CNA- 8 or 12 hour shifts. Management makes schedules. Try to accommodate block scheduling if it works in the schedule. - Weekend Requirements: Every other. - Holiday Requirements: Every other. - Call or Standby Requirement: No - Floating Requirements: Could float between units. Try to keep on the same unit if working a stretch. - Scrub Color: Any color of professional scrubs. - Unit/Department Specific Orientation: One 12 hour shift on the floor. IF you are not comfortable/confident after that shift, let management know. - Do you have a Separate COVID Unit: Intermixed. - Do you use PAPR or N95: Both
