Title: Social Worker – (LCSW or LICSW) Duration: 13-weeks Location: 100 North Beacon Street Boston, MA Schedule: 8:00am – 4:30pm Hours Per Week: 40 hours Start: 11/12/24 • Assess patients and families and provides intervention and treatment to support patients. • Address psychosocial needs and patterns of adjustment throughout the hospitalization and collaborates with the multidisciplinary team to formulate treatment plans to maximize emotional, social and physical well-being. • Provide information and education regarding health care and community resources. • Completes MDS in conjunction with interdisciplinary team. • Must hold current LCSW or LICSW license in MA. • Skilled nursing facility or rehab experience preferred. • Orientation begins 11/12/24.
