Openings in San Ysidro, El Cajon, National City, and Chula Vista! Requirements; 2 years of experience minimum as an NP Experience with Geriatric patients Certifications: Nurse Practitioner License, or Licensed Independent Practitioner Preferred: Nationally Certified Family Nurse Practitioner. Day to Day: Under the direction of the San Diego PACE Medical Director, the PACE Nurse Practitioner provides comprehensive medical care to the PACE participant. The PACE NP attends to the acute needs of the PACE participant, while managing the chronic medical problems, and paying attention to the preventive and rehabilitative care required by the PACE participant. The PACE NP incorporates the entire interdisciplinary team as appropriate to provide the medical care in a holistic manner, as is the hallmark of the delivery of care to the PACE participant.
