Clinical Pharmacist (Home Infusion) - Syracuse, NY Job Type: Full-time/Part-time (on-site) Shifts: M-F (8:30a-5:00pm) (on-call - occasionally) Pay: $58 - $78 hourly Responsibilities: Providing consultation, patient assessment, patient management, and therapeutic monitoring pertaining to infusion therapy. Preparation of IV medications within a USP 797 and 800 compliant cleanroom Performs admixture and preparation, labeling, and dispensing of medications and solutions Communicating/collaborating with patients, physicians and nursing agencies Preparation of patient IV medications and deliveries Participation in all aspects of pharmacy operations including staff supervision, production, dispensing, distribution, record-keeping, and inventory control Medication Management Oversee Pharmacy Technicians Utilize your clinical expertise to collaborate with physicians on antibiotic ordering and monitoring, TPN order development, monitoring and modifications, appropriate methods of infusion administration for home Utilize EMR for documentation Utilize external EMR systems for optimal clinical patient monitoring Requirements: Valid NYS Registered Pharmacist license Compounding/ Infusion/ IV Therapy experience required Benefits: 401(k) 401(k) matching Dental insurance Flexible schedule Health insurance Health savings account Life insurance Paid time off Professional development assistance Retirement plan
