What You Will Do
Oversee an interdisciplinary team of community-based clinicians that deliver quality patient care from admission to discharge.
Manage, empower, and motivate team members in a dynamic work environment.
Proactively execute administrative functions to achieve seamless patient care including the review of clinician caseloads, staffing needs, recruitment process, and assigned scheduling of team members.
Use and learn tools and systems, problem solve new and different situations, and escalate issues timely to leadership to achieve organizational success and improved clinical outcomes.
Licenses and Certifications:
License and current registration to practice as a Registered Professional Nurse or Physical Therapist in New York State, required
Education:
Bachelor’s degree in nursing or physical therapy from a program approved by the New York State Department of Education or equivalent work experience or
Master's degree in nursing or physical therapy or equivalent work experience, required
Work Experience:
Minimum of three years nursing experience and one year home health experience or
Minimum of three years of physical therapy experience, required
Clinical management experience, preferred
