Job Opportunity: Home Health Physical Therapist

Overview

Schedule

• Work Hours: 5x8 Days, 08:00-16:30; Monday-Friday
• Floating Requirement: Potential to float to Wytheville area 2-3 times per week (approximately 40 miles from the office).

Requirements

• Education: Doctor of Physical Therapy (DPT) or Master's degree required.
• Experience: Home health experience is required; one year of experience accepted.
• Licensure: Virginia Physical Therapy license required (can apply immediately upon offer).
• Certifications:

  • Basic Life Support (BLS) required.
  • Additional specialized certifications preferred but not required.

Training

• Training on the basics of the Epic system for evaluations, routine visits, and discharges will be provided.

Additional Information

• Mileage Reimbursement: Available for travel to floating locations.
• No Call or Overtime: The position does not require on-call duties or overtime.

Documentation and Compliance

• I-9 or Attestation: Required.
• Drivers License/State ID: Must be provided within 48 hours of signing.
• E-Verify or Attestation: Required.
• Residency Declaration Form: Required if working on a compact license.
• Certifications: Various certifications may be required based on unit specifications (e.g., ACLS, BLS, PALS, NRP, TNCC).
• Health Documents:

  • Physical Exam within 1 year of start.
  • Fit Testing for masks within 1 year of start.
  • Negative TB test or chest X-ray within 1 year of start.
  • Vaccination documentation for Rubeola, Mumps, Rubella, Hepatitis B, and Varicella.
  • Negative drug screen within 30 days of start.

Background Check

• A 7-year background check is required within 1 year of start, covering all locations lived/worked in the past 7 years.

References

• Two clinical references are required (hospital evaluations also accepted).
