Required Credentials and Certifications

  • Driver's License/State ID: Required for onboarding.

  • Drug Screening: Minimum 10-panel test required within 60 days of start.
  • Color-blind Test: Must be completed annually.
  • Agency Attestation: Required for onboarding.
  • SAM/EPLS Check: Required for onboarding.
  • Resume: Required for submission.
  • References: Two references required for submission.
  • Name Verification Affidavit: Optional for onboarding if there are name discrepancies.
  • BLS Certification: Required from American Heart Association or Red Cross.
