Overview

Facility: Resurrection Medical Center, part of Prime Healthcare Foundation, Inc.
Employment Type: LTA (Long-Term Assignment)
Contract Length: 12 months

Shift Availability

Shifts:
- 12:00 PM - 8:30 PM
- 2:30 PM - 11:00 PM
Must be available for at least one of these shifts.

Required Credentials and Certifications

Driver's License/State ID - Required for onboarding.
Prime OIG - Required for onboarding.
Prime Drug Screening (minimum 10-panel test) - Required for onboarding.
Prime Color-blind test - Required for onboarding.
Prime Agency Attestation - Required for onboarding.
Prime SAM/EPLS - Required for onboarding.
License/Certificate Verification - Required for onboarding.
COVID-19 Vaccine - 2 vaccines and booster required or declination completed.
MMR (Measles, Mumps, Rubella) - Proof of 2 vaccines or positive titer showing immunity required.
Fit Test - Annual respirator fit test required.
Hepatitis B - Required for onboarding.
Influenza Vaccine - Required from October 1 to May 31.
TB Test - Required for onboarding.
Varicella - Vaccination/titer showing immunity required.
Tdap - Required for onboarding.
Annual TB Questionnaire - Required for onboarding.
Social Security Card - Required for onboarding.
Physical/Health Statement - Must state clear to work, required for onboarding.
Prime Core Competencies Exam - Must score 80% or higher.
Prime Skills Checklist - Required for submission.
Prime Criminal Background Check - Required for onboarding.
Prime Badge Photo - Clear headshot required for onboarding.
Prime Form I-9 (Employment eligibility verification) - Required for onboarding.
Prime Job Description - Required for onboarding.
Prime COA - Required for onboarding.
DCI/BVP Certification - Required for onboarding.

Additional Information

A notarized name affidavit is required for any name discrepancies between credentials (optional for submission).
