Job Description

• Hours: 36 or 48 hours per week
• Schedule: Day/night rotating 50/50, every other weekend, and holidays
• Experience Required: 1+ years of travel and level 1 experience

Required Credentials

1. BLS Certification

   • Accepted from AHA, Red Cross, CHSF, or Military.
   • Required for onboarding.

2. License/Certificate Verification

   • Primary source verification of professional license and/or certification.
   • Required for onboarding.

3. Fit Test

   • N95 fit test required within 365 days of start date.
   • No onsite fit testing available.

4. MGB Medication Exam

   • 80% passing score required within 365 days.
   • Accepted from any testing site.

5. COVID-19 Vaccine

   • Completed primary vaccination series required.
   • No exemptions or waivers considered.

6. MMR Vaccination

   • Documentation of two measles, two mumps, and one rubella vaccine required.
   • Proof of immunity via blood test accepted.

7. Influenza Vaccine

   • Required during flu season (Sept 15 - April 30).
   • No exemptions or waivers considered.

8. TB Test

   • Documentation of TB skin test or negative QuantiFERON TB Test required within 3 months of start date.

9. Health Statement

   • Physician statement signed by MD/DO/NP/PA required within 365 days of start date.

10. MGB Criminal Background Check

    • Required for onboarding.

11. MGB Massachusetts CORI

    • Criminal history check required for the state of MA.

12. MGB Resume

    • Must include past 7 years of work experience and education.

13. MGB References

    • Minimum of 2 recent references, including at least 1 supervisor.

14. MGB Skills Checklist

    • Required for submission.

15. MGB Confirmation of Assignment/POI Form

    • Required for all initial assignments and extensions.

16. MGB Education Verification

    • Verification of highest level of education achieved.

17. MGB SSN Trace

    • Must verify addresses/aliases for the past 7 years.

18. MGB National Sex Offender Check

    • Required for onboarding.

19. MGB Exclusion Checks

    • Search OIG, GSA, and MassHealth & NH Medicaid exclusion lists.

Additional Information

• Documentation must be uploaded as PDF where specified.
• All health history and credential requirements must be updated as necessary.
• Strongly preferred: Documentation of Hepatitis B vaccination or proof of immunity.
