Required Credentials and Health History

1. MMR Vaccination

   • Documented proof of two MMR vaccines or positive IGG titer. No declinations accepted.

2. Fit Test

   • Required for all travelers. Preferred to be completed prior to start, can be completed onsite.

3. Hepatitis B

   • Documentation of vaccination series completion or signed declination.

4. Influenza Vaccine

   • Required during flu season (October 1, 2024 - March 31, 2025). Exemptions accepted with mask requirement.

5. PPD/IGRA

   • 2 Step PPD or IGRA (Quantiferon/Tspot) required.

6. Varicella

   • Documented proof of two Varicella vaccines or serological evidence of immunity.

7. Tdap

   • Valid for 10 years from the administered date.

8. Health Statement

   • Statement of good health or completed physical exam prior to first assignment.

9. Driver's License/State ID

   • Must match address on file.

10. Ascension Skills Checklist

    • Clinical skills checklist to be completed within one year of start.

11. Ascension 12 Panel Drug Screen

    • Must show proof of all 12 drugs tested.

12. Ascension Badge Photo

    • Forward-facing headshot with a white or blue background.

13. Education Verification

    • Verification of education relevant to clinical work.

14. BLS Certification

    • Must be AHA certified.

15. Nursys Verification

    • Verification within 30 days prior to start.

16. License/Certificate Verification

    • Current license verification required.

17. Ascension Criminal Background Check Results

    • Must be completed prior to first assignment.

18. Ascension Employment Verification

    • Third-party employment verification for the past seven years.

19. Ascension Core Competencies

    • HIPAA, Joint Commission - NPSG, OSHA Topics training required.

20. MOAB Moderate Risk Certification

    • Required for de-escalation training.

Additional Information

• Onboarding Requirements: Various pre-hire modules and training must be completed prior to clearance.
• Exit Survey: To be completed within 72 hours at the end of the assignment.
