Required Certifications

• BCLS (Basic Cardiac Life Support)
• NIH (National Institutes of Health) Stroke Certification
• ACLS (Advanced Cardiac Life Support)

Important Notes

• The facility has a strict attendance policy, allowing only 2-3 call-offs per contract.

Health Document Requirements

• Vaccinations:

  • Tetanus/Diphtheria/Tdap (preferred within 10 years)
  • Negative TB or Chest X-Ray
  • Rubeola (Measles), Mumps, Rubella (MMR)
  • Hepatitis B
  • Varicella
  • Drug Testing (10 panel within 30 days of start)
  • Flu Seasonal Vaccination/Documentation for 2024-25
  • COVID-19 vaccination records or signed declination form (not required)

Background Check

• A 7-year criminal check is required within a year of start and updated annually.

References

• Proof of 2 clinical references (hospital evaluation also accepted) is required.
