Job Opportunity: RN PACU

Overview

Required Credentials and Certifications

  1. BLS Certification: AHA, Red Cross, CHSF, or Military accepted.
  2. License/Certificate Verification: Primary Source Verification of professional license and/or certification required within 30 days of start date.
  3. Fit Test: N95 fit test required within 365 days prior to start date, updated annually.
  4. MGB Medication Exam: 80% passing score required within 365 days.
  5. COVID-19 Vaccine: Completed primary series or up-to-date vaccine required; no exemptions considered.
  6. MMR Vaccination: Documentation of two measles, two mumps, and one rubella vaccine or proof of immunity required.
  7. Hepatitis B: Strongly preferred documentation of vaccination or proof of immunity.
  8. Influenza Vaccine: Required during flu season (Sept 15 - April 30); no exemptions considered.
  9. TB Test: Documentation of TB skin test or negative QuantiFERON TB Test required within 3 months of start date.
  10. Health Statement: Physician statement required within 365 days of start date.
  11. Tdap Vaccine: Preferred documentation within 10 years.
  12. Criminal Background Check: Required within 45 days prior to start date.
  13. Massachusetts CORI: Attestation required within 30 days prior to start date.
  14. Resume: Must include past 7 years of work experience and education.
  15. References: Two recent references required, including at least one supervisor.
  16. Skills Checklist: Required for submission.
  17. Confirmation of Assignment/POI Form: Required upon acceptance of assignment.
  18. Education Verification: Must verify highest level of education achieved.
  19. SSN Trace: Required to list addresses/aliases for the past 7 years.
  20. National Sex Offender Check: Required within 30 days of start and annually.
  21. Exclusion Checks: Required to search OIG, GSA, and MassHealth & NH Medicaid exclusion lists.
