Employment Details

Required Credentials and Certifications

  1. BLS (Basic Life Support)

    • AHA or ARC only
    • Required for onboarding
  2. Nursys License

    • Nursing license must be current within 30 days of start
    • Required for onboarding
  3. ACLS (Advanced Cardiovascular Life Support)

    • AHA only
    • Required for onboarding
  4. License/Certificate Verification

    • Primary source verification of professional license and/or certification
    • Required for onboarding
  5. FMOLHS Skills Checklist

    • Unit specific to the contractor’s scope of practice within one year prior to start date; updated annually
    • Required for onboarding
  6. FMOLHS Competency Exam

    • Must have a passing score of 80% or higher within the year of start
    • Required for onboarding
  7. FMOLHS Core Competencies

    • Must be current within a year prior to start with passing score of 80% or higher; updated annually

  8. FMOLHS CPI (Crisis Prevention Intervention)

    • As required per specialty/Unit & Facility
    • Required for onboarding

Personal Health History Requirements

  • MMR (Measles, Mumps, Rubella)

    • Proof of 2 immunizations, positive titer, or declination accepted
    • Required for onboarding
  • Fit Test

    • Must be one of the 3 mask fit types (N95- 3M 1860, 1860s, or 1870+)
    • Must be completed within 1 year prior to start; updated annually
    • Required for onboarding
  • Hepatitis B

    • Proof of 3 immunizations, positive titer, or declination accepted
    • Required for onboarding
  • Influenza Vaccine

    • Proof required during flu season; only medical/religious exemptions accepted
    • Required for onboarding
  • TB Test

    • Proof of negative blood test or skin test within one year prior to start
    • Required for onboarding
  • Health Statement

    • Statement signed and dated within 1 year of start stating traveler is able to work without restrictions
    • Required for onboarding
  • Varicella (Chickenpox)

    • Proof of 2 immunizations, positive titer, or declination accepted
    • Required for onboarding
  • Tdap (Tetanus, Diphtheria, Pertussis)

    • Proof of vaccination within past 10 years or declination accepted
