Job Description

• Provide physical therapy services to patients in their homes who have physical impairments, functional limitations, or disabilities due to injury, disease, or other causes.
• Collaborate with patients, families, physicians, and other health professionals to identify needs and coordinate services.
• Patients may range in age from birth to over 100 years.
• Weekend rotations are required once every 6 weeks.
• Ability to travel to assigned cases is necessary.

Requirements

• Licensure: Must have a Physical Therapist Professional license in Massachusetts.
• Experience: Minimum of 1 year of home care experience required.

Credentialing Requirements

• MGB Badge Photo: Submit a color photo taken in the past 6 months.
• MVR Check: Required for all home care positions.
• License/Certificate Verification: Primary source verification of professional license required within 30 days of start date.
• Fit Test: N95 fit test required within 365 days of start date.
• Nurse Aide Registry Check: Required by MA DPH for all employees.
• COVID-19 Vaccine: Completed primary vaccination series required.
• MMR Vaccination: Documentation of measles, mumps, and rubella vaccinations or proof of immunity required.
• Hepatitis B: Record of vaccinations or proof of immunity preferred.
• Influenza Vaccine: Required during flu season.
• TB Test: Documentation of TB skin test or negative QuantiFERON TB Test required.
• Health Statement: Physician statement required within 365 days of start date.
• Background Checks: Criminal background check and Massachusetts CORI required.
• Resume: Include past 7 years of work experience with city and state.
• References: Two recent references required, including at least one supervisor.
• Skills Checklist: Required for submission.
• Confirmation of Assignment/POI Form: Required upon acceptance of assignment.

Preferred Qualifications

• Tdap Vaccine: Documentation preferred within 10 years.
• Additional Health Screening: TB symptom survey if positive PPD.
