Minimum # Years Clinical Experience Required: 1 yr. HH
Minimum # Years Clinical Experience in Specialty Required: 1-year HH
Nurse Patient Ratio by shift: Average caseload 18-22 clients for Case Manager; equivalent 5 visits per 1.
Patient Population (Diagnoses): Cardiac, pulmonary, diabetes, wound care, post-surgery, orthopedics, neurological impairment, palliative care, dementia, CVA, SCI, COPD, Parkinson's, ostomy, catheters, MVA, etc.
Age Range: 30-100
Special Equipment: laptop, iPhone and clinical items provided including trunk bin/visit bag-VS needs, wound care, catheters, etc.

Job requirements

Certifications Required: BLS, OASIS experience
Certifications Preferred: OASIS
Prior Travel Experience Required: No
Floating Required: No

Physical therapist needed for Home Health.

Schedule information

Shift Hours: 8am - 5pm
Self-Scheduling: Yes, within guidelines and needs of department
Weekend Requirements: currently Sat/Sun a month with 2 days to comp during week

# Days of Orientation: 2-3 days

Unit specific information

Specialty Testing for Unit/Facility: Unit onboarding prior to arrival on computer
Documentation System: Netsmart (training is part of orientation)
Unit Specific Procedures and Protocols: Phlebotomy, catheters, IV, coag-u-check, port access, wound vacs, TPN, Feeding tubes, drains, nebulizer.
Respiratory available 24/7 for support: Yes
Anesthesia available 24/7 for support: Yes

Additional Details

Rate: $90 Local Rates: $85: 51-100 Mile $80: 50 miles or less
Travel Experience is preferred; Experience in Home Health setting is preferred.
Covid and Flu declinations accepted.
Covid vax or declination must be included with file upon submission.
Public Transportation is NOT accessible in this area, please ensure you have reliable transportation, personal vehicle or rental vehicle are strongly recommended.
No block schedules for any reason, vehicles should be equipped with all season or snow tires during the months of November – April.

I really enjoy working for Access Healthcare. My recruite, Deina, goes to bat for me. The agency pays better than other agencies. They will match pay as well. Their CEO, Diego is very down to earth and transparent. They will also pay for your certifications and RN license fees. You just show them the receipt and they are able to refund you on the next paycheck.
