- BLS
- License/Certificate Verification
- LTA for a CT Technologist position at Mount Carmel East.
- American Heart Association certification required.
- License/Certificate Verification within 30 days of start.
- Fit Test required.
- 9 Panel Drug Screen within 30 days of start.
- Trinity Code of Conduct Form on file.
- Standard I9 completed by the agency.
- Trinity Health Confidentiality of Patient and Proprietary Information and Information System Security Agreement on file.
- Trinity Integrity and Audit Services Form.
- Specialty exam completion within a year.
- Trinity Color Vision test required.
- 2 Step PPD OR IGRA (Quantiferon/Tspot) required.
- COVID-19 Vaccination proof or exemption (not valid for CA or IL).
- MMR documentation required.
- Hepatitis B series vaccination required.
- Influenza Vaccine proof or exemption (not valid for CA or IL) within flu season dates.
- Varicella documentation required.
- Tdap documentation required.
- Physical/Health Statement from the last year.
- Trinity Conflict Of Interest Disclosure Form on file.
- Trinity Criminal Background Check within 30 days of start.
- Trinity Education Verification on file if job requires specific certification(s).
- Trinity EPLS and Trinity GSA/OIG reports on file within 30 days.
- NSO check required.
- Trinity System Access Form completed one week before start.
- Shift: Friday, Saturday, Sunday from 7 PM to 7:30 AM
- Agency must comply with documentation requirements for onboarding.

Their CEO, Diego is very down to earth and transparent. They will also pay for your certifications and RN license fees. You just show them the receipt and they are able to refund you on the next paycheck.
