Physical Therapist - Home Health - (PT - HH)
StartDate: ASAP Available Shifts: 8 D Pay Rate: $2186.87 - $2396.25
Position Title: PHYSICAL THERAPIST Total number of positions needed: 1 All of the info below must be completed before I can open the req. 1. Who will be the DOO approving the contractors time each week? Maame Duah 2. What is the iCIMS req # corresponding to this requisition? 2024-32335 How long is the contract assignment? 13 weeks is the norm: 6 -7 weeks possibly (covering our FT PT who is out on leave) Schedule of assignment hours? M – F 8am – 5pm Will there be any weekend rotation? If so, how often? N/A Will there be any on call? If so, how much? N/A What region are you in? Home health or Hospice? Home health What territory this contractor will be asked to cover? Frederick County What’s creating the request? covering a Full-Time PT who is out on emergency sick leave, unsure of return at this time, but possibly 6 to 7 weeks Which location will this person report to for their assignment- add info below Full address: 7360 Guilford Dr. St 201A, Frederick, Md, 21704 OU# (location code) 5018 About how many miles per week will the contractor travel? 300-400 What are the weekly units of productivity expectations? Ex: 25/30 ? 25-30 What EMR systems will the consultant be using: HCHB Do you want the candidate to have experience in have home health, hospice or both ? Yes, in Home Health
