Physical Therapist - Home Health - (PT - HH)
StartDate: 12/1/2024 Available Shifts: 8 D Pay Rate: $1730.08 - $1993.35

PT - Home Health needed asap for 13 weeks Covid vaccine required. REQUIREMENTS: 1 reference from the past 12 months. Home Health Physical Therapist: 3 years of experience required. HH experience but will consider those with strong SNF as well. BLS required. MT PT License required. Mileage radius is about 30 miles. Sometimes we have a couple outliers. However average daily mileage is about 40. 200 mile radius rule. ALL RTO or schedule request must be on submission-HCF will not be approving RTO or schedule request after submission.
