Occupational Therapist - Home Health - (OT - HH)
StartDate: ASAP Available Shifts: 8 D Pay Rate: $1806.00 - $2081.00

OT needed asap Graduate of an Accredited School of Occupational Therapy Maintains NBCOT Membership Requirements Currently licensed and in good standing in the State of Colorado Previous home care experience and Occupational Therapy experience preferred. Valid driver’s license and auto insurance, ability to drive to and from client homes BLS EMR= Sansio Home Solutions Performs initial and ongoing client assessments and applies appropriate goals and interventions to resolve identified problems. Provides Occupational therapy services to client according to physician orders and regulations. Assures physicians are notified in a timely manner of changes in client status and physician’s orders are obtained as needed for changes to the plan of care. Identifies and obtains/makes appropriate modifications to equipment/environment to meet client needs. Provides teaching and instruction to client/caregiver to promote physical and emotional health. Attends required staff meetings and case conferences. Contributes to the coordination of services through effective and timely communication with other members of the health care team.

Required Qualifications
Graduate of an Accredited School of Occupational Therapy
Maintains NBCOT Membership Requirements
Currently licensed and in good standing in the State of Colorado
Previous home care experience and Occupational Therapy experience preferred.
Valid driver’s license and auto insurance, ability to drive to and from client homes
BLS
