Physical Therapist - Home Health - (PT - HH)

Hello, Position Title: PHYSICAL THERAPIST Total number of positions needed: 1 All of the info below must be completed before I can open the req. 1. Who will be the DOO approving the contractors time each week? Sumit Arora 2. What is the iCIMS req # corresponding to this requisition? 2024-28402 How long is the contract assignment? 13 weeks is the norm: 13 weeks Schedule of assignment hours? M – F 8am – 5pm? Yes Will there be any weekend rotation? If so, how often? No weekend rotation but may be asked to pick up the odd weekend SOC if willing to do so. Not mandatory. Will there be any on call? If so, how much? No call What region are you in? Home health or Hospice? Home Health What territory this contractor will be asked to cover? Hampton and surrounding cities. Area is very compact and accessible. Very nice area to cover! What’s creating the request? - work load Ect.. Growth of the Care center What is the best rate you can offer for this need?Which location will this person report to for their assignment- add info below Full address: One Enterprise Pkwy, suite 120 Hampton VA 23666 Ph: OU# (location code) 1712. Please also give access to 1753 (Williamsburg) About how many miles per week will the contractor travel? 150 to 200 What are the weekly units of productivity expectations? Ex: 25/30 ? 30 What EMR systems will the consultant be using: HCHB Do you want the candidate to have experience in have home health, hospice or both ? HH
