Physical Therapist (PT) - Skilled Nursing FacilityAre you a dedicated Physical Therapist looking for an opportunity to make a real impact? Join our team in providing exceptional care in a skilled nursing facility (SNF) setting, where your expertise will improve the lives of patients and support their journey to better mobility and independence.What You’ll Do:

Deliver patient-centered physical therapy services tailored to individual needs.
Develop and implement personalized treatment plans to maximize physical function and recovery.
Collaborate with a multidisciplinary team to ensure comprehensive care for residents.
Track and document patient progress in compliance with industry standards.
Educate patients and caregivers on exercises and strategies to maintain mobility.

What You’ll Bring:

Active Physical Therapist license in good standing.
Graduation from an accredited Physical Therapy program.
Strong interpersonal and communication skills.
Passion for working in a skilled nursing facility and helping patients achieve their goals.
Experience in geriatrics or a SNF setting (preferred but not required).
