Job Summary:
We are seeking a dedicated and experienced Physical Therapist to join our rehabilitation team. The successful candidate will be responsible for evaluating patients developing personalized treatment plans and implementing physical therapy interventions to help patients achieve their rehabilitation goals. The Physical Therapist will work closely with other healthcare professionals to provide coordinated and effective care.
Key Responsibilities:Conduct thorough assessments of patients' physical conditions and functional limitations.Develop individualized treatment plans based on patient assessments and goals.Implement physical therapy interventions including exercises manual therapy and modalities to improve patients' mobility strength and functional abilities.Educate patients and their families on treatment plans home exercises and ways to improve overall wellness.Monitor and document patients' progress adjusting treatment plans as needed to ensure optimal outcomes.Collaborate with interdisciplinary team members including physicians occupational therapists and nursing staff to ensure coordinated and comprehensive care.Participate in team meetings case conferences and continuing education opportunities to stay current with best practices and advancements in physical therapy.Maintain accurate and timely documentation of patient care in compliance with regulatory and facility standards.Adhere to ethical and professional standards set by the American Physical Therapy Association APTA and state licensing board.
Qualifications:Doctor of Physical Therapy DPT degree from an accredited program.Current state licensure or eligibility for licensure as a Physical Therapist in California.Proven experience as a Physical Therapist preferably in a post-acute or rehabilitation setting.Strong clinical skills in assessment treatment planning and implementation.Excellent communication and interpersonal skills to effectively interact with patients families and healthcare team members.Ability to work independently and as part of a collaborative team.Commitment to ongoing professional development and staying current with advancements in physical therapy.CPR certification preferred.
Physical Requirements:Ability to lift and move patients equipment and supplies.Prolonged periods of standing walking and performing physical therapy interventions.Manual dexterity and strength to perform therapeutic exercises and manual therapy techniques.
