Physical Therapist needed for home health setting surrounding Colchester, VT

13 weeks

Minimum 1 year home health experience required

Average caseload 18-22 clients for Case Manager; equivalent 5 visits per 1.

Mileage is usually limited to a specific area of the county, e.g., 30-mile radius, unless emergent need.

Documentation system: Netsmart

Must have VT license in hand (or proof of application) prior to submitting.
