Position will cover visits in Easley and Pickens for our hospice and home health division.

As a Hospice & Home Health Aide / Certified Nursing Assistant (CNA), here’s a big-picture view of what you’ll do:
- Provide care for seniors and individuals with illnesses or chronic conditions in their homes.
- Assist with daily activities such as bathing, toileting, dressing, grooming, hygiene, nail/skin care, eating and nutritional intake.
- Support with ambulation, transfers and range of motion exercises.
- Monitor vital signs, intake/output, and blood glucose levels and document any changes.
- Work with the care team to follow and update the patient’s care plan.
- Observe patient and notify supervisor of issues requiring a change in care, medication or medical equipment.
- Follow the patient’s care plan to provide appropriate services.
- Assists patients in safely performing incidental/instrumental activities of daily living.
- Provide personal care, including bathing, oral hygiene, hair care and skin care.
- Help with dressing, feeding, and changing incontinent products as needed.

Requirements
A few must-haves for Hospice & Home Health Aide / Certified Nursing Assistants (CNA):
- High school diploma (or equivalent) and active Nursing Assistant Certification in South Carolina.
- Valid driver’s license, auto insurance, and reliable transportation.
- Compassionate nature, good communicator and ability to lift up to 50 lbs.
