Position will cover visits Pickens and Oconee counties for our home health and hospice division.

As a Home Health / Hospice Aide / Certified Nursing Assistant (CNA), here’s a big-picture view of what you’ll do:

• Provide care for seniors and individuals with illnesses or chronic conditions in their homes.
• Assist with daily activities such as bathing, toileting, dressing, grooming, hygiene, nail/skin care, eating and nutritional intake.
• Support with ambulation, transfers and range of motion exercises.
• Monitor vital signs, intake/output, and blood glucose levels and document any changes.
• Work with the care team to follow and update the patient’s care plan.
• Observe patient and notify supervisor of issues requiring a change in care, medication or medical equipment.
• Follow the patient’s care plan to provide appropriate services.
• Assists patients in safely performing incidental/instrumental activities of daily living.
• Provide personal care, including bathing, oral hygiene, hair care and skin care.
• Help with dressing, feeding, and changing incontinent products as needed.

Requirements

A few must-haves for Home Health Aide / Certified Nursing Assistants (CNA):

• High school diploma (or equivalent) and active Nursing Assistant Certification in South Carolina
• Valid driver’s license, auto insurance, and reliable transportation
• Compassionate nature, good communicator and ability to lift up to 50 lbs.
