Job Opportunity: RN Case Management

Position Details

Contract Length: 13 weeks

Job Description

The RN will coordinate the management of care, ensuring optimal resource utilization, service delivery, and compliance with external review agencies. Responsibilities include:

Comprehensive assessment and care planning
Implementation and evaluation of individual patient needs
Enhancing patient management quality and satisfaction
Promoting continuity of care and cost-effectiveness
Serving as a patient advocate through resource utilization and discharge planning

Facility Requirements

Certifications:
- BLS certification (must be through the American Heart Association; RQI certifications will not be accepted)
Experience:
- Minimum of 2 years RN Case Management experience in a long-term acute care setting
- Experience in discharge planning, clinical case management, and utilization review
- Must have experience working at a Kindred Hospital
Licensure:
- Must possess a valid Kentucky RN license

Work Schedule

Shifts: Monday - Friday, 8:00 AM - 4:30 PM
On-call/Weekends: None
