Job Summary
We are seeking a compassionate and skilled Registered Nurse to join our Home Care team. The RN will provide high-quality nursing care to patients in the comfort of their homes, ensuring their health needs are met while promoting independence and well-being. This role requires exceptional clinical expertise, strong communication skills, and the ability to work autonomously in a variety of home settings.

Key Responsibilities
Patient Care:
Conduct comprehensive patient assessments and develop individualized care plans.
Administer medications, treatments, and procedures as prescribed by the healthcare provider.
Monitor patients' conditions and document changes in health status.
Educate patients and their families on managing illnesses, medications, and preventive health practices.
Coordination and Collaboration:
Work closely with physicians, therapists, and other members of the healthcare team to ensure continuity of care.
Coordinate with family members or caregivers to create a supportive environment for the patient.
Refer patients to community resources or other healthcare services as needed.
Documentation:
Maintain accurate and thorough medical records in compliance with regulatory and organizational standards.
Document assessments, interventions, and patient progress in the electronic medical record (EMR).
Quality and Safety:
Adhere to infection control protocols and maintain a safe environment for patients and caregivers.
Participate in quality improvement initiatives to enhance home care services.
Qualifications
Education:

Graduate of an accredited nursing program (Associate's or Bachelor's degree in Nursing).
Bachelor of Science in Nursing (BSN) preferred.
Licensure/Certification:

Active Registered Nurse (RN) license in the state of practice.
BLS (Basic Life Support) certification required.
Additional certifications in home health care are a plus.
Experience:

Prior experience in home care, medical-surgical, or community health nursing is preferred.
New graduate RNs with strong clinical skills and a passion for home care may be considered.

Strong organizational and time management skills to handle multiple patients effectively.
Exceptional interpersonal and communication skills for working with diverse populations.
Ability to work independently while collaborating with a multidisciplinary team.
Proficiency in using electronic medical records (EMR) systems.
