EOW - 1 winter holiday required
No triage in the position, this will be for overflow and expansion to beds in the hallways
Summary:
Provides direct and indirect patient care in the Emergency Department. The triage nurse will assist with triage in the EMR in-basket and nurse telephone line, nurse case management, collaboration with clinical teams, clinicians, and other departments as necessary, and provide quality patient care in compliance with local, state, and federal regulations and accreditation standards.
Communicates with physicians about changes in patient's clinical condition including hemodynamic monitoring, results of diagnostic studies and symptomatology. Employees can respond quickly and accurately to changes in condition or response to treatment. Additionally, employees can perform general nursing duties in all departments with adequate supervision. Participates in performance improvement and CQI activities.
Responsibilities:
This nurse works with all primary care practices to assist with triage duties and related tasks.
Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties: Monitors and responds to triage calls in person, through the nurse phone line and/or via an EPIC in basket.
Talks directly to patients on the telephone and directs them to emergency rooms/urgent care centers, dispenses home care advice, or schedules appointments with health care clinicians during office hours.
Determines urgency of seeing patient based on brief assessment and familiarity with patient's condition and history.
Appropriately prioritizes patient care (e.g., seeks assistance as needed from appropriate resources, reassigns personnel as necessary, recognizes need for emergency intervention, etc.).
Provides thorough, timely, and accurate documentation of all pertinent data, therapeutic interventions and patient responses including assessment, care plan, patient goals, and interventions according to established standards.
Provides timely and thorough transfer of nursing care on assigned patients to other health care team members to ensure integration of services and continuity of patient care.
Assumes accountability for remaining current regarding standards of care established by the profession, regulatory and accreditation bodies and hospital policies and procedures.
Sends patients with high-risk chief complaints such as chest pain, abdominal pain, or severe headaches to emergency departments immediately or arranges for an ambulance.
Provides appropriate home health advice to patients who do not need to go directly to the emergency room, and schedules appointments with provider as necessary.
Ensure accurate documentation for all conversations and treatments are recorded in patient's electronic medical record.
Arranges appointments for patients who do not need to go to the emergency room but need to see a physician.
Proactively identifies risk to patient safety and identifies opportunities to reduce medical/health care errors (i.e., appropriate reporting of errors/incidents).
Identifies ethical issues in care and implements actions to address.
Qualifications:
Graduate of accredited school of nursing. BSN preferred
Current nursing registration in Pennsylvania/compact state required.
Demonstrated clinical competence in the specialty area of nursing practice preferred.
AHA BLS // BCLS required.
