Job Summary and QualificationsThis position supports the mission of The Medical Center of Aurora/Centennial Medical Plaza of optimizing the quality of life of all those served by expanding beyond the traditional focus on medical needs to the needs of the whole person.
The Heart and Vascular Center at The Medical Center of Aurora focuses on the cardiovascular patient population, including arrhythmias, heart failure, ACS, cardiogenic shock, pulmonary embolus response team, ECMO, eCPR, and valvular dysfunction, among other cardiac related needs .
Under general supervision, in accordance with our mission and core values, utilizing advanced clinical knowledge and skills, the Cardiovascular Nurse Coordinator will collaborate with physicians and Advanced Practitioners to provide oversite of direct care and care facilitation for patients requiring advanced mechanical support and/or critical care coordination. Prioritizing the patient and family experience and offering exceptional customer service are of upmost importance. Job duties include, but are not limited to:
• Coordinate initial work up and diagnostic testing for patient and families. Also assists families coming from out of town in finding logistics and amenities.
• Obtain all outside imaging, and other medical records, as appropriate
• Coordinate additional testing i.e. Cath, TEE, CT/MRI
• Facilitate direct contact between specialist and referring physician regarding treatment plan
• Have all testing available for review by cardiologist
• Evaluate best practice and evidence-based research/protocols and collaborate with
physician/providers
• Facilitate and coordinate multidisciplinary meetings, track attendance, minutes, follow up items
• Participate in development of treatment or management plan, attend daily rounds
• Patient and family support and education
• Facilitate direct communication between specialist and referring physician regarding treatment plan
and progress
• Provide clinical knowledge for authorization of procedures/pharmaceuticals
• Provide patients and families access to timely care
• Initiates and coordinates outreach programs
• Initiates and coordinates education for hospital staff
• May take call for patients needing off-hours phone support
• Participates follow-up visits and coordinates further follow-up
• Initiates and coordinates outreach programs
• Evaluates, initiates and coordinates education for hospital staff
• Tracks required data for quality
• Attends weekly conferences where applicable
• Enthusiasm for new treatment options for high risk patients
• Consistently supports compliance and the principles of responsibility by maintaining confidentiality,
protecting the assets of the organization, acting with integrity, reporting noncompliance, adhering to
applicable federal and state laws and regulations and accreditation and licensure requirement
What qualifications you will need:- Current licensure in the State of Colorado as a Registered Nurse, or current active multistate Registered Nurse license.
- BLS required from AHA or American Red Cross.
- Graduate of an accredited school of nursing – BSN required
- Background in cardiac or pulmonary preferred
- Strong focus on customer service and patient experience; ability to use Microsoft WORD, Excel, and data entry; ability to multi-task
