Job Details:

• Pay: $ (to be updated)

• Physical Therapist

• Position Open: 1

• Duration: 13 weeks

• Shift:  Day 5x8-Hour (08:00 - 16:30)

• Guaranteed Hours: 40 hours weekly

• Ratios: 1:1 (*May visit up to 6 patients per day; 30 productivity points standard) 

• First-Timers Accepted: Yes 

• Weekend REQ: No

• Is on-call REQ? No 

• Will clinician float within scope to meet facility needs? No 

• Open to accommodating block schedule? No 

• Locals Accepted: Yes; Local radius rule: 25-50 miles

• Pending License Accepted: No

• Oasis is preferred, but not required

• Traveler is expected to have their own transportation.

• Type of Facility: Home Health 

• Attire / Scrub Color: Any basic color scrubs 

• Charting: EPIC 

• Parking Cost: Free 

• Mileage Reimbursement: IRS 

 

Requirements:

• Active Unrestricted AK License

• Active BLS via American Heart Association

• Certs REQ: BCLS

• Years of Experience REQ: 1 year of skilled home health experience is required

• Skills REQ: Experience in home exercise programs, lifting and transfer, patient transport and treatment, and scheduling appointments

• Must have skilled Home Health experience

• Must be able to provide at least 1 supervisory references
