Physical Therapist

Guaranteed Hours: 36 hours weekly

Actively monitors and amends patient visit schedule to maximize productivity and patient need
-
Performs a comprehensive (OASIS and PT) assessments of assigned patients including, but not limited to psychosocial, educational, safety and risk factor parameters, as evidenced by clinical documentation and in accordance with federal and state regulations, organizational and departmental policies and standards of care, and APTA guidelines.
Requirements:

Active Unrestricted VT License

BLS certification - required
-
Must have at least 1 year of experience

Must be able to provide at least 1 supervisory references
