Job Details:

• Pay: $2640 per week

• Duration of assignment: 13 weeks

• Positions Open: 1

• Shift: 8hr Days (*This facility cannot accommodate 4x10s or 3x12s.*)

• Ratios: 1:1 (*May visit up to 6 patients per day; 30 productivity points standard)

• First-Timers Accepted: No

•  Weekend REQ: 1 Weekend Shift Per Contract

• Is On-Call REQ?: No 

• Will this traveler need to float between like units?: No 

• Open to accommodating block schedule?: No 

• Locals Accepted: Yes; Considered "local" within 50 mile radius, will accept locals at the same rate - 

• Pending License Accepted: Yes 

• RTO Restrictions: Confirm with hiring manager 

• Guaranteed Hours: Guaranteed Work Week (per contract)

• Special Requirements - Traveler is expected to have reliable transportation and no additional scheduling restrictions. All travel pairs will be required to have their own transportation.

• Type of Facility: Home Health 

• Attire / Scrub Color: Any basic color scrubs 

• Charting: EPIC

• Parking Cost: Free 

• Oasis is preferred, but not required
   

Requirements:

• Active Unrestricted CA License

• Active BLS certification through American Heart Association

• Must have at least 1 year of experience of skilled PT experience (willing to accept clinicians without prior home health experience)

• Must be able to provide 1 supervisory reference
