Overview

Required Certifications and Credentials

1. BLS (Basic Life Support)

   • Required for all direct patient care personnel.
   • Attach scanned copy of certification (with expiration date).

2. ACLS (Advanced Cardiovascular Life Support)

   • Required for RNs in CCU, ED, PACU, MICU, SICU.
   • Attach scanned copy of certification (with expiration date).

3. License/Certificate Verification

   • Must be dated within 30 days of start.
   • Attach scanned online verification page.

4. Fit Test

   • Required for onboarding.

5. Trinity 9 Panel Drug Screen

   • Must be completed within 30 days of start date (preferred within 14 days).
   • Must include specific substances (NO THC).

6. Trinity Code of Conduct Form

   • Must have page 27 signed form on file.

7. Trinity I9

   • Standard I9 completed by the agency.

8. Trinity Confidentiality Agreement

   • Must have page 5 signed form on file.

9. Trinity Integrity and Audit Services Form

   • Required for onboarding.

10. Trinity Specialty Specific Competency Testing

    • Attach copy of specialty exam completed within a year.

11. Trinity Color Vision

    • Required for onboarding.

12. 2 Step PPD OR IGRA (Quantiferon/Tspot)

    • Required for onboarding.

13. COVID-19 Vaccine

    • Proof of vaccine, declination, or exemption required.

14. MMR (Measles, Mumps, Rubella)

    • Attach proof of vaccination.

15. Hepatitis B

    • Must have first vaccination prior to start; proof required.

16. Influenza Vaccine

    • Proof of vaccination required for the flu season.

17. Varicella (Chickenpox)

    • Attach proof of vaccination.

18. Tdap (Tetanus, Diphtheria, Pertussis)

    • Upload current, non-expired vaccine.

19. Physical/Health Statement

    • Provide documentation from an exam within the last year.

20. Trinity Profile

    • Resume, Skills Checklist within a year, and 2 Professional References required.

21. Trinity Conflict Of Interest Disclosure Form

    • Must have page 1 signed form on file.

22. Trinity Criminal Background Check

    • Federal, Nationwide, County Criminal from last 7 years required.

23. Trinity Education Verification

    • Attach completed report of highest level of academic education.

24. Trinity EPLS (Excluded Parties List System)

    • Attach scanned report dated within the past 30 days.

25. Trinity GSA/OIG (Government Services Administration/Office of Inspector General)

    • Attach scanned result dated within the past 30 days.

26. Trinity NSO (National Sex Offender Registry)

    • Must be dated within 30 days of start.

27. Trinity System Access Form

    • Must be completed and uploaded as soon as possible.
