Position Summary

The Registered Nurse (RN) will implement a full-time professional nursing service program to patients in their homes under the supervision of the Clinical Services Supervisor, adhering to agency standards and policies.

Essential Functions and Responsibilities

1. Initiate and provide medically prescribed skilled nursing care and/or health counseling to contribute to treatment, rehabilitation, and symptom management.
2. Conduct initial visits to assess the patient’s physical and emotional status, needs, and resources through observation, interviews, and record analysis.
3. Prepare the nursing plan of care in accordance with the physician’s treatment plan, based on identified problems, and provide patient education using the Care Plan/Standard of Care.

Requirements

• Experience: 2+ years of experience required
• Skills:

  • Infusion and wound care experience required
  • OASIS experience preferred
  • Home care experience preferred
  • Travel experience preferred

Additional Information

• Contract Length: 13 weeks
• Case Load: Typically 7 visits per day
• EMR System: Home Care Home Base
• Practice: Point of Care
• Dress Code: Matching scrubs
• Mileage Reimbursement: $0.34 per mile
