Job Description

The Social Worker (LCSW) will be responsible for managing the healthcare needs and achieving quality and cost outcomes for all patients within Berkshire Medical Center. This role involves:

• Utilizing a multidisciplinary team approach to negotiate, procure, coordinate, and monitor services and resources needed by patients.
• Reviewing patients at admission and throughout their hospital stay to ensure appropriate levels of care and resource utilization.

Additional Requirements

• Education:

  • Master's in Social Work required
  • LCSW (Licensed Clinical Social Worker) required
  • LICSW (Licensed Independent Clinical Social Worker) strongly preferred
• Experience:

  • 3+ years of clinical experience in acute care settings (internal medicine, surgery, orthopedics, mother/child, cardiology, oncology, psychiatry)
  • Experience in rehabilitation settings, home health, and long-term care is considered.
• Skills:

  • Knowledge of CMS regulations, appeal rights, and DPH regulations
  • Familiarity with Kepro QIO (appeals)
  • Understanding of regulatory and accreditation organizations (e.g., Joint Commission, DPH, HealthGrades)
  • Strong organizational, analytical, and interpersonal skills
  • Ability to maintain confidentiality and work independently
  • Knowledge of local community resources and reimbursement processes (Medicare, Medicaid, third-party payers)
  • Proficiency in data management and computer literacy
