Job Description

• Responsibilities:

  • Work in a Trauma Level 1 setting for both pediatric and adult patients.
  • Perform portable imaging, assist in surgery, and operate GE and Phillips portable machines.
  • Weekend assistance required.

Required Credentials and Qualifications

Mandatory Credentials

• License/Certificate Verification: Valid permanent license required; temporary licenses not accepted.
• MMR: Positive MMR titers from within the last 15 years; booster requirements apply.
• Fit Test: Must be completed within one year prior to start.
• Hepatitis B: Proof of vaccination series or signed declination required.
• Varicella: Positive titers from within the last 15 years; booster requirements apply.
• Tdap: Proof of vaccination within the past 10 years; declinations not accepted.
• ARRT Certification: Required.
• UHS I-9 Attestation: Must provide completed I-9 or attestation.
• UHS Ohio Fingerprints: Background check requirements based on residency duration in Ohio.
• UHS Education Verification: Verification of highest level of education required.
• UHS Employment Verification: Verification of employment within the last 5 years required.
• UHS Competency Exam: Must score at least 80%; to be completed within one year prior to start.
• Physical/Health Statement: Pre-employment exam statement required.
• Influenza Vaccine: Proof of vaccination or approved exemption required.
• 2-Step PPD or IGRA: Negative results required within the last 6 months.
• UHS Criminal Background Check Results: Must be completed within 6 months prior to start.
• UHS Drug Screen: Must be completed within 60 days prior to start.

Optional Credentials

• COVID-19 Vaccine: Preferred but not required.
• Annual Negative PPD: If not done within the past year.
• UHS National Database Criminal Background Check: For Ohio residents.
