Required Credentials and Certifications

1. BLS Certification

   • Must be AHA certified.

2. Nursys Verification

   • Required within 30 days prior to start.
   • Complete FACIS Questionnaire for any disciplinary actions.

3. License/Certificate Verification

   • Current license and primary source verification required within 30 days prior to start.

4. Ascension Medical Surgical Exam

   • Must be completed prior to start and updated every 3 years with a minimum score of 80%.

5. Ascension Color Vision

   • Completed prior to start and updated every 3 years.

6. Ascension Core Competencies

   • Must be completed within a year of start and updated yearly with a minimum score of 80%.

7. NIH Certification

   • Group A updated yearly, Group B-F updated every 2 years.

8. Ascension EKG Exam

   • Must be completed with a passing score of at least 80% prior to clearance.

9. Ascension Onboarding Requirements Education Attestation

   • Attestation required after completing pre-hire modules.

10. Health Statement

    • Statement of good health or completed physical exam required prior to first assignment, updated annually.

11. Fit Test

    • Required for all travelers, preferred to be completed prior to start.

12. Vaccination Documentation

    • MMR, Hepatitis B, Influenza, Varicella, and Tdap vaccinations required.

13. 2 Step PPD or IGRA

    • Documentation required prior to start.

14. Ascension Skills Checklist

    • To be completed within one year of start and updated annually for specialty.

15. Ascension 12 Panel Drug Screen

    • Must be completed within 30 days prior to start.

16. Ascension Education Verification

    • Verification of education relevant to clinical work required within 90 days of start.

17. Ascension Criminal Background Check Results

    • Must be completed within 30 days of first assignment and updated annually.

18. Ascension Employment Verification

    • Third-party employment verification for the past seven years required.
