Required Credentials and Certifications

1. Driver's License/State ID: Required for onboarding.

3. Drug Screening: Minimum 10-panel test required within 60 days of start.
4. Color-blind Test: Must be completed annually.
5. Agency Attestation: Required for onboarding.

8. Name Verification Affidavit: Required for any name discrepancies.
9. BLS Certification: Must be from American Heart Association or Red Cross.

18. Fit Test: Annual respirator fit test required.

21. TB Test: Required for onboarding.
22. Varicella Immunity: Vaccination or titer required.

27. Core Competencies Exam: Must score 80% or higher.
