Job Description

• Schedule: Rotating weekends and holidays; 4 days a week with potential for extended hours based on case loads.
• Call Requirement: 14 days of call in a 6-week period.
• Procedures:

  • Angioplasty
  • TAVR-Watchman's
  • Peripheral vascular EP
  • Carotid EP
• Experience Required:

  • Must have experience in a busy cath lab.
  • Experience with structural hearts is required.
  • EPIC and MERGE experience preferred.

Personal Health History

• COVID-19 Vaccine: 2 vaccines and booster required or declination completed.
• MMR: Proof of 2 Vaccines or Positive Titer showing immunity.
• Fit Test: Annual respirator fit test required.
• Hepatitis B: Vaccination or declination required.
• Influenza Vaccine: Required from October 1 to May 31; declinations not accepted at certain facilities.
• TB Test: Required.
• Varicella: Vaccination/titer showing immunity required.
• Tdap: Vaccination required; declinations not accepted at certain facilities.
• Annual TB Questionnaire: Required.
• Physical/Health Statement: Must be completed annually.

Testing Requirements

• Prime Basic Pharmacology Exam: Must score 80% or higher.
• Prime Specialty Exam: Must score 80% or higher.
• Prime Core Competencies Exam: Must score 80% or higher.
• Prime Skills Checklist: Required.

Additional Notes

• A national criminal background check is required within one year of the start date.
• A clear headshot photo is required for the badge.
• A notarized name affidavit is needed for any discrepancies between credentials.
