Requirements for Healthcare Professionals
Licensure and Certifications
- Nursys Verification: Must be completed within 30 days prior to start.
- Current License Verification: Required for all licensed professionals within 30 days prior to start.
- Ascension Color Vision: Completed prior to start and updated every 3 years.
- Ascension Core Competencies: Must be completed within a year of start and updated yearly with a minimum score of 80%.
- Ascension EKG Exam: Must complete with a passing score of at least 80% prior to clearance.
Health and Safety
- MMR Vaccination: Documented proof of two MMR vaccines or positive IGG titer required.
- Fit Test: Required for all travelers, preferably completed prior to start.
- Hepatitis B Documentation: Proof of vaccination series completion or signed declination required.
- Influenza Vaccine: Required during flu season (October 1 - March 31). Exemptions accepted with mask requirement.
- TB Screening: 2 Step PPD or IGRA required prior to start.
- Varicella Vaccination: Documented proof of vaccination or immunity required.
- Tdap Vaccination: Must be valid for 10 years from the administered date.
Professional Documentation
- Resume or Profile: Required for submission.
- Professional References: At least two references needed prior to first assignment.
- Criminal Background Check: Must be completed within 30 days of first assignment and updated annually.
- Education Verification: Must be completed within 90 days of start.
- Driver's License/State ID: Must not be expired and match the address on file.
Additional Onboarding Requirements
- Health Statement: Statement of good health or completed physical exam required prior to first assignment.
- Ascension Skills Checklist: To be completed within one year of start and updated annually for specialty.
- 12 Panel Drug Screen: Must be completed within 30 days prior to start.
- Ascension Badge Photo: Forward-facing headshot required.
Other Information
- Exit Survey: To be completed within 72 hours at the completion of the assignment.
