Job Description

• Experience required in both adult and pediatric care.

Required Certifications and Credentials

1. BLS (Basic Life Support)

   • Certification from the American Heart Association required.
   • Attach scanned copy of certification (with expiration date).

2. ACLS (Advanced Cardiovascular Life Support)

   • Certification from the American Heart Association required for RNs in CCU, ED, PACU, MICU, and SICU.
   • Attach scanned copy of certification (with expiration date).

3. License/Certificate Verification

   • Must be dated within 30 days of start.
   • Attach scanned online verification page with a machine-generated date indication.

4. Trinity 9 Panel Drug Screen

   • Must be completed within 30 days of start date, preferred within 14 days.
   • Must include specific substances (e.g., Cocaine, Amphetamines, Opiates, etc.).

5. Trinity Code of Conduct Form

   • Must have page 27 signed form on file.

6. Trinity I9

   • Standard I9 completed by the agency.

7. Trinity Confidentiality Agreement

   • Must have page 5 signed form on file.

8. Trinity MVR (Motor Vehicle Report)

   • Must be dated within 30 days of start.

9. Trinity Skills Checklist

   • Required for clinical contractors.

10. 2 Step PPD OR IGRA (Quantiferon/Tspot)

    • Must be completed within 30 days of start.

11. COVID-19 Vaccine

    • Proof of vaccine, declination, or exemption required.

12. MMR (Measles, Mumps, Rubella)

    • Proof of vaccination required.

13. Hepatitis B

    • Proof of vaccination required.

14. Influenza Vaccine

    • Proof of vaccination required.

15. Varicella (Chickenpox)

    • Proof of vaccination required.

16. Tdap (Tetanus, Diphtheria, Pertussis)

    • Proof of vaccination required.

17. Physical/Health Statement

    • Documentation from an exam within the last year.

18. Trinity Profile

    • Resume, Skills Checklist within a year, and 2 Professional References required.

19. Trinity Conflict Of Interest Disclosure Form

    • Must have page 1 signed form on file.

20. Trinity Criminal Background Check

    • Must be dated within 30 days of start.

21. Trinity Education Verification

    • Attach completed report of highest level of academic education.

22. Trinity EPLS (Excluded Parties List System)

    • Attach scanned report dated within the past 30 days.

23. Trinity GSA/OIG (Government Services Administration/Office of Inspector General)

    • Attach scanned result dated within the past 30 days.

24. Trinity NSO (National Sex Offender Registry)

    • Must be dated within 30 days of start.

25. Trinity System Access Form

    • Must be completed and uploaded as soon as possible.
