Required Credentials and Certifications

8. Ascension Core Competencies

   • Must complete within a year of start and updated yearly with a minimum score of 80%.

9. NIH Certification

   • Group A updated yearly, Group B-F updated every 2 years.

10. Ascension EKG Exam

    • Must complete with a passing score of at least 80% prior to start.

11. MMR (Measles, Mumps, Rubella)

    • Documented proof of two vaccines or positive IGG titer required.

12. Fit Test

    • Required for all travelers, preferred to be completed prior to start.

13. Hepatitis B

    • Documentation of vaccination series completion or declination required.

14. Influenza Vaccine

    • Required for flu season (October 1, 2024 - March 31, 2025). Exemptions accepted with mask requirement.

15. Health Statement

    • Statement of good health or completed physical exam required prior to first assignment.

16. 2 Step PPD or IGRA

    • Required prior to start with annual updates.

17. Varicella

    • Documented proof of vaccination or immunity required.

18. Tdap (Tetanus, Diphtheria, Pertussis)

    • Must be valid for 10 years from administered date.

19. Ascension Profile

    • Resume or profile required.

20. Professional References

    • At least two professional references required.

21. Criminal Background Check

    • Must be completed within 30 days of first assignment and updated annually.

22. Education Verification

    • Must be completed within 90 days of start.

23. Ascension Confirmation of Assignment

    • Provide upon initial assignment and update upon each extension.

24. Ascension 12 Panel Drug Screen

    • Must be completed within 30 days prior to start.

25. Ascension Badge Photo

    • Forward facing headshot required.
