10. Ascension EKG Exam

    • Must be completed with a passing score of at least 80%.
    • Only two attempts permitted.

11. MMR (Measles, Mumps, Rubella)

    • Documented proof of two MMR vaccines or positive IGG titer required.

12. Fit Test

    • Required for all travelers, preferred to be completed prior to start.

13. Hepatitis B

    • Documentation of vaccination series completion or declination required.

14. Influenza Vaccine

    • Required for flu season (October 1, 2024 - March 31, 2025).
    • Exemptions accepted, but masks required if not vaccinated.

15. Health Statement

    • Must be completed within 30 days of first assignment.

22. 12 Panel Drug Screen

    • Must be completed within 30 days prior to start.

23. Ascension Education Verification

24. Ascension Confirmation of Assignment

    • Provide upon initial assignment and update upon each extension.
