Job Description

• Seeking a Radiation Therapist with experience in Aria and Truebeam.

Required Credentials and Certifications

1. Personal Health History:

   • 2 Step PPD OR IGRA (Quantiferon)
   • Trinity Specific Covid Questionnaire
   • COVID-19 Vaccine (proof of vaccination required)
   • MMR (proof of vaccination required)
   • Hepatitis B (proof of vaccination required)
   • Influenza Vaccine (proof of vaccination required)
   • Varicella (proof of vaccination required)
   • Tdap (proof of vaccination required)
   • Physical/Health Statement (fit to work)
   • Fit Test

2. Professional Credentials:

   • Trinity Child and Adult Dependent Abuse Training (IA Only)
   • Trinity IA Sing Report (Iowa's Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check)
   • Trinity OFAC (must be completed within 30 days of start)
   • Trinity Skills Checklist (for clinical contractors)
   • Trinity Profile (resume, skills checklist, 2 professional references)
   • Trinity Code of Conduct Form
   • Trinity Confidentiality of Patient and Proprietary Information Agreement
   • Trinity Conflict Of Interest Disclosure Form
   • Trinity Criminal Background Check (dated within 30 days of start)
   • Trinity Education Verification (highest level of academic education)
   • Trinity EPLS (dated within past 30 days)
   • Trinity GSA/OIG (dated within past 30 days)
   • Trinity NSO (dated within 30 days of start)
   • Trinity System Access Form
   • BLS (American Heart Association certification)

3. Drug Screening:
