Job Description

• Schedule:

  • 1st week: One 12-hour shift, two 10-hour shifts
  • 2nd week: Four 10-hour shifts
  • Weekends: Every other weekend, 3rd shift (10 hours)
  • Total weekly hours: Approximately 36 (may vary)
• Call: None
• Experience Required:

  • Siemens and/or GE experience preferred
  • Minimum of 2 years experience
  • Familiarity with EPIC
• Floating: May need to float onsite within skill set or to sites within 40 miles if needed

Required Credentials

1. BLS Certification: Must be AHA certified.
2. ARRT Certification: Required.
3. WI State License: Current and valid.
4. Ascension Specialty Exam: Must complete prior to first assignment and updated every 3 years.
5. MMR Vaccination: Documented proof of two MMR vaccines or positive IGG titer; no declinations accepted.
6. Fit Test: Required for all travelers; preferred to be completed prior to start.
7. Hepatitis B Vaccination: Documentation of vaccination series completion or declination.
8. Influenza Vaccine: Required during flu season (October 1 - March 31); exemptions accepted with mask requirement.
9. PPD/IGRA Testing: 2-step PPD or IGRA (Quantiferon/Tspot) required.
10. Varicella Vaccination: Documented proof of vaccination or immunity.
11. Tdap Vaccination: Must not be expired.
12. Health Statement: Statement of good health or completed physical exam prior to first assignment.
13. Driver's License/State ID: Must match address on file.
14. Criminal Background Check: Must be completed within 30 days of first assignment.
15. Education Verification: Verification of education relevant to clinical work.
16. Professional References: At least two professional references required.
17. Drug Screen: 12-panel drug screen required within 30 days prior to start.

Additional Information

• Competency Testing: HIPAA, Joint Commission - NPSG, OSHA topics must be completed within a year of start and updated yearly.
• Onboarding Requirements: Various onboarding documents and attestations must be completed and submitted as specified.
