Job Description

• Work in a Trauma Level 1 setting for both adult and pediatric patients.
• Responsibilities include performing X-rays in surgery, portables, and the emergency room.
• Rotating weekends required.

Required Credentials and Qualifications

Mandatory Credentials

1. License/Certificate Verification

   • Primary Source Verification of professional license and/or certification required within 30 days of start and updated upon expiration. Temporary licenses are not accepted.

2. MMR (Measles, Mumps, Rubella)

   • Positive titers only from within the last 15 years. Specific booster requirements apply.

3. Fit Test

   • Must be completed within 1 year of start and updated annually.

4. Hepatitis B

   • Proof of full series or at least two vaccines from the series, or signed declination.

5. Varicella

   • Positive titers only from within the last 15 years. Specific booster requirements apply.

6. Tdap (Tetanus, Diphtheria, Pertussis)

   • Proof of vaccination within the past 10 years; declinations not accepted.

7. ARRT Certification

   • Required for the position.

8. UHS I-9 Attestation

   • Completed I-9 or attestation that completed I-9 is on file.

9. UHS Ohio Fingerprints

   • Background checks required based on residency duration in Ohio.

10. UHS Education Verification

    • Verification of highest level of education; attestations not allowed.

11. UHS Employment Verification

    • Verification of employment within the last 5 years; attestations not allowed.

12. UHS Competency Exam

    • Must score at least 80% within 1 year prior to start.

13. Physical/Health Statement

    • Pre-employment exam statement signed, indicating ability to work without restrictions.

14. UHS Criminal Background Check Results

    • Must be completed within 6 months prior to start.

15. UHS OIG Search

    • To be completed within 6 months prior to start.

16. UHS Drug Screen

    • Must be completed within 60 days of start date.

Optional Credentials

• COVID-19 Vaccine

  • Proof of vaccine series or approved exemption form preferred.

• Annual Negative PPD

  • If not completed within the past year.

• UHS Module Completion Certificate

  • Safety and Compliance Training modules to be completed prior to clearance.
