Requirements

Credentials and Documentation

1. Influenza Vaccine: Proof required from October to March or approved exemptions.
2. Driver's License/State ID: Non-expired photo ID.
3. 2-Step TB Test: Negative results within the last 6 months or acceptable alternatives.
4. Resume: Must include education, work history, and 2 professional references from the last 2 years.
5. UHS Badge Photo: Headshot with a plain background, uploaded as JPEG.
6. Criminal Background Check: Must be completed within 6 months prior to start.
7. OIG Search: To be completed within 6 months prior to start.
8. Drug Screen: 10-panel drug screen completed within 60 days of start.
9. Safety and Compliance Training: Completion certificate required.
10. BLS Certification: American Heart Association certification required for clinical positions.
11. License/Certificate Verification: Primary source verification required within 30 days of start.
12. COVID-19 Vaccine: Proof preferred, not required.
13. MMR: Positive titers required from within the last 15 years.
14. Fit Test: Completed within 1 year of start.
15. Hepatitis B: Proof of vaccination or signed declination required.
16. Tdap: Proof of vaccination within the past 10 years.
17. I-9 Attestation: Completed I-9 or attestation required.
18. Education Verification: Highest level of education must be verified.
19. Employment Verification: Verification of employment within the last 5 years required.
20. Skills Checklist: Clinical skills checklist to be completed within 1 year prior to start.
21. Competency Exam: Must score at least 80%, completed within 1 year prior to start.
22. Core Competencies: Must complete within 1 year prior to start.
23. Physical/Health Statement: Pre-employment exam statement required, completed within one year of start date.
24. National Board of Respiratory Care Certification: Required.
